Everolimus for advanced renal cell carcinoma after previous treatment

  1. I confirm that an application has been made by and the first cycle of systemic anti-cancer therapy is to be prescribed by a consultant specialist specifically trained and accredited in the use of systemic anti-cancer therapy
  2. I confirm that the patient has biopsy proven renal cell carcinoma
  3. I confirm that the patient has progressed during or after treatment with vascular endothelial growth factor targeted therapy
  4. I confirm that the use of everolimus will be as per the Summary of Product Characteristics (SPC)
